The Ageing and Health (A&H) team look after predominantly frail patients with complex medical and social needs, often involving multiple co-morbidities. These patients are looked after on the below wards:
The A&H team also have a presence at the front door within the Acute Assessment Unit (AAU), with a core of A&H consultants, frailty assessment nurses and a clinical development fellow who work with colleagues to provided initial support for frail patients and ensure completion of comprehensive frailty assessment at point of admission.
There is a daily frailty huddle at lunchtime to discuss patients from the frailty ward round and bring together an MDT to help facilitate safe discharge; this includes A&H consultants, frailty nurses, representation from health and social care partnerships and AHP colleagues.
The A&H department meet on a Friday lunchtime for a weekly education session which is led by a member of the team, including junior doctors.
